1. Correction. Something Cyclops shows is factually wrong: a label, a flag, a creator link, an outcome or a number. Point to the transactions or addresses that show the right answer.
2. Takedown. Something should not be shown at all: a page that names a person, content that infringes a right, or a statement about a creator that the chain data does not support.
3. Privacy. It concerns you personally, for example an address publicly tied to your name. These are listed in the log without their subject and are handled privately.

Scan results describe public chain data and are never a rating; a request cannot make a token “pass”. Disagreeing with a reading of the data is an appeal, filed in Cyclops Eyes.
